Comments:
An unannounced monitoring inspection was conducted today from 9:25 am until 11:45am. There were 6 children (14) points present with 2 caregivers. The physical plant, menu's, attendance records, evacuation drill logs, shelter in place drill logs, children's records, caregiver record's and household member record's and emergency supplies were inspected. The children were observed having a music lesson, hand washing and doing an art project. Medications were also inspected. Areas of non-compliance were cited on the Violation Notice. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on review of 2 of 5 children's records, the provider failed to maintain up to date records at the family day home for each child enrolled. Evidence: Child #1 and #2 do not have documentation of authorization for pick up.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-240-A
Description: Based on inspection of the playground, the family day home failed to ensure that areas and furnishings of the family day home outside shall be maintained in a clean, safe and operable condition. Evidence: The playground toys are not clean.

Plan of Correction: We will clean the outside toys.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to keep potentially hazardous materials in areas inaccessible to children. Evidence: The soap in the upstairs bathroom states "keep out of reach of children."

Plan of Correction: Removed and replaced during inspection.

Standard #: 22VAC40-111-500-D
Description: Based on inspection of the playground, the family day home failed to ensure that equipment with moving parts that might pinch or crush children's hands or fingers shall not be used unless they have guards or covers. Evidence: One of the springs on the playground Hobby Horse is exposed and does not have a cover.

Plan of Correction: I will replace the cover for the spring on the Hobby Horse.
