Comments:
An unannounced monitoring inspection was conducted today from 12:00pm until 2:00pm. There were 3 children (7 points) present with 3 caregivers. The physical plant including outside play area, children's records, caregiver's records, attendance logs, evacuation logs and emergency supplies were inspected. The children were observed napping, and playing with floor toys. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-111-760-A
Description: Based on inspection of the first aid kit, the family day home failed to ensure that all minimum supplies are in the first aid kit. Evidence: The first aid kit is missing the first aid instruction manuel.

Plan of Correction: I will get a manual for the first aid kit.

Standard #: 22VAC40-111-780-B
Description: Based on review of 1 of 3 children's files, the provider failed to ensure that annually the provider will review with the parent the emergency contact information to ensure the information is correct. Evidence: Child #1 does not have documentation of an annual review of the emergency contact information in the file.

Plan of Correction: Will contact annual review with parent and document in child's file.

Standard #: 22VAC40-111-50-A
Description: Based on review of attendance logs, the family day home provider failed to keep a written record of children in attendance each day. Evidence: The provider did not have an attendance record on the date of inspection 7/22/2019.

Plan of Correction: I will document attendance daily.

Standard #: 22VAC40-111-230-A
Description: Based on caregiver's record review, 1 of 3 caregivers, the family day home provider does not have documentation of each caregivers's orientation. Evidence: Caregiver #2 does not have documentation in their file of orientation.

Plan of Correction: I will find the documentation of the caregiver's orientation and place it in the file.
