An unannounced monitoring inspection was conducted today between 10:00 am and 12:30pm. There are six children enrolled; however, no children were present today. Physical plant, children's records, caregiver's records, household member's records, emergency plans, evacuation logs, cribs and emergency supplies were inspected. Description: Based on review of children's files (1 of 3), the family day home provider failed to maintain up to date records for each child enrolled. Evidence: Child #2's file is missing documentation for the second emergency contact.

Plan of Correction: I will get information from parent and put in child's file.
