An unannounced monitoring inspection was completed on 07/23/2019 from 9:45 A.M.-12:30 P.M. At the time of the inspection there were twelve children, two caregivers, and one visiting child with a guardian present. There was a total of sixteen points. The records for five children, two caregivers, one household member, four injury reports, emergency plan, first aid kit and emergency supplies, outside play area, and required posted information were reviewed. The children were observed during outdoor water play, free choice activities that included drawing, knitting and dramatic play. Large group gathering time which included finger plays, songs, and storytelling, transition times, behavioral guidance, handwashing, snacktime, and outdoor playtime. Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, the provider failed to ensure that an updated tuberculosis screening was updated every two years from the date of the first screening. Evidence: 1. Caregiver #1's tuberculosis screening on file was dated 06/05/2017. 2. The provider verified the date of the tuberculosis screening on file. Plan of Correction: Caregiver #1 went on 07/22/2019 to have the tuberculosis test administered, and will return to on 07/24/2019 to have it read. Caregiver #1 was out of the country for the month of June 2019.
