Comments:
An unannounced mandated monitoring inspection was conducted today, 7/24/2019. There were 6 children present (16 points) with 2 care givers. Children were observed building with colored magnetic panels, watching Paw Patrol, playing super heroes with capes, and singing in circle time. Reviewed 2 caregiver files, 2 children's files, attendance, emergency plan, emergency evacuation drills, and physical plant. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on record review, written parent authorization noting any known adverse reactions was not obtained at least annually when topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent are used. Evidence: Child#1 and Child#2 did not have documentation of parent authorization for sunscreen.

Plan of Correction: I will add that to my handbook for parents.

Standard #: 22VAC40-111-60-A
Description: Based on record review, provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: Child#2 did not have documentation of an annual review of child's emergency contact information.

Plan of Correction: I will have the parent review the information.
