Technical Assistance:
The new backyard play piece is labeled to be used by children ages 3-10 only. The provider shall ensure that children beyond this age range do not use this piece of equipment. The supplemental preservice training for all subsidy caregivers must be completed by 3/1/20.

Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 5 enrolled children and 2 resident children, totaling 15 points. Children's records, the indoor/outdoor physical plant, children's nutrition, programming, emergency preparedness items, and caregiver training were inspected today. The children were observed creating art projects, playing with toys, and eating lunch. The children were served pasta with tomato sauce and cheese, bananas, and milk for lunch today. This inspection was completed between approximately 10:45am and 12:20pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on a review of children's topical ointment products, it was determined that these items were not labeled as required. Evidence: Three containers of diaper ointment were not labeled with the child's name.

Plan of Correction: I will label them today.

Standard #: 22VAC40-111-920
Description: Based on observations, it was determined that the provider has not posted the menu for the current one week period. Evidence: The provider serves lunch and snacks to all children in care. The provider did not a post a menu for the current week.

Plan of Correction: I will remember to post the menu at the start of each week.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1 - Child record A and record B do not include documentation showing that each parent has reviewed their child's contact information within the past year. 2 - Child record C does not include documentation showing that the provider has viewed the child's original proof of identification.

Plan of Correction: 1 - Both parents will be asked to review their child's file. 2 - I saw the birth certificate but forgot to document it. I will update the documentation.

Standard #: 22VAC40-111-500-C
Description: Based on observations, it was determined that one piece of stationary outdoor playground equipment has not been placed at least 6 feet from an obstacle. Evidence: The provider recently installed a large piece of outdoor play equipment for the preschoolers and school-age children. There is not at least 6 feet of open space between the three swings on this piece of equipment and the chain link fence surrounding the play piece. There is only approximately 3 feet of space between the swings and the fence.

Plan of Correction: I will remove the swings before the children use the playground again. I will also move the play piece further away from the fence.
