Comments:
An unannounced monitoring inspection was conducted from 12:00pm-1:30pm. There were 9 children (21 points) in the direct care of the Provider and the Assistant Provider. A sample size of 4 children's records, 4 caregiver/household member records, evacuation drills, emergency materials, attendance, the first aid kit, and the home were reviewed. The children were observed playing with toys, building with blocks, reading books, and taking a nap. If you have any questions, please contact me at shahana.green@dss.virginia.gov . Thank you.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, 1 out of 4 records reviewed were missing documentation of a physical examination. Evidence: 1. Child #2 (start date: 4/8/2019) does not have documentation of a physical examination on file.

Plan of Correction: The parent was contacted during the inspection and will bring the physical tomorrow.

Standard #: 22VAC40-111-180-A
Description: Based on a review of household member records, 1 out of 2 adult household members do not have a current tuberculosis screening. Evidence: 1. Household member #1 has documentation on file for the last tuberculosis screening being conducted on 5/22/2017.

Plan of Correction: The household member will call today and will get a new TB screening.
