Comments:
The licensing inspector conducted an unannounced monitoring inspection on 7/16/2019 from 9:35pm to 11:23pm. The children were playing outside upon the inspector?s arrival. Today, there were six children, totaling 13 points, present with one caregiver. The caregiver was observed interacting with the children. Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate the inspector interacted with children. The menu was posted. The provider does not administer medication at this time. The provider does not provide transportation. The provider has pets. Five children?s records, one caregiver record, and one household member's record was reviewed during the inspection. Last emergency drill: 6/05/2019 Last shelter-in-place drill: 5/24/2019 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, the provider did not obtain all required information for each child's record. Evidence: 1. The record of child #6 (start date: 7/8/19) did not contain the name/address/phone number for two emergency contacts, physician and parents. The name/ policy number of the child's medical insurance. Information on allergies, intolerances to food, medication, or any other substances, and actions to take in an emergency. Names of persons authorized to pick up the child. Information regarding chronic physical problems pertinent developmental information, and any special accommodations needed. First date of attendance. Parent's signed acknowledgment of the receipt of the information required by 22 VAC 40-111-70 and written authorization for emergency medical care. 2. The record of child #1 (start date: 8/1/16) did not contain documentation of the child's proof of age and identity.

Plan of Correction: 1. Received child #6's record on 7/16/19. 2. I asked child #1's parent for the birth certificate and they said they would bring it to me.
