Technical assistance was provided regarding becoming a subsidy provider. Go to www.dss.virginia..gov research "how to become a subsidy provider", complete the form and submit.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-12:30pm. There were 11 children (25 points) supervised by 3 direct caregivers. The home, 2 staff records, 11 children?s records, 2 household member?s records, evacuation drills, programing and policies were inspected. There were no medications on site today. Children were observed engaging freely, participating in gym time, eating snack and and eating lunch. Diapering procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observation, topical skin products are not labeled with child's name. Evidence: Diaper ointments are not labeled with child's name.

Plan of Correction: I will label them and make sure when a parent bring them in they have a name on them.

Standard #: 22VAC40-111-100-B
Description: Based on review of 11 children's records, 1 does not have a physical prior to first day of attendance. Evidence: Child # 5, start date 2/11/19, does not have documentation of a physical examination.

Plan of Correction: I will get one from the parent.

Standard #: 22VAC40-111-60-B
Description: Based on review of 11 children's, 4 records are missing an annual update of the emergency contacts. Evidence: Children #1, 2, 3 & 4 do not have annual updates to their emergency contacts.

Plan of Correction: I will have the parents review the emergency contacts.

Standard #: 22VAC40-111-90-A
Description: Based on review of 11 children's records, 1 does not have documentation of an immunizations within 30 days of first day of attendance. Evidence: Child # 5 does not have an immunization record on file.

Plan of Correction: I will get the immunization record from the parent.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous materials were accessible to children. Evidence: Wipes, (keep out of reach of children) were on a low shelf, making them accessible to children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-570-A
Description: Based on observation, provider was over 16 points. Evidence: Upon arrival one caregiver was alone with 11 children, 23 points. The second caregiver was changing a diaper, in another room with the door closed.

Plan of Correction: There was another caregiver in the other room changing a diaper. I will make sure we are always in compliance.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use. Evidence: After 2 different diaper changes the diapering mat was not cleaned or sanitized. Mat must cleaned with soap & water and wiped dry, then sprayed with bleach and water and air dry.

Plan of Correction: We will make sure we follow diapering procedures.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of household 2 household member records, 2 do not have documentation of an updated sworn statement. Evidence: Household members #1& 2 have expired sworn statements on file, expired 6/4/19.

Plan of Correction: We will sign new sworn statements this evening.
