Comments:
An unannounced monitoring inspection was conducted today between 9:50am and 12:30 pm. There were 11 children (27 point) present; however, 3 of the children reside in the home. Ratio's, children's files, caregiver's files, menu's and physical plant were inspected. The children were observed singing songs, playing with toys, hand washing and eating snack. Violations:
Standard #: 22VAC40-111-920
Description: Based on observation the family day home provider failed to ensure that the menu for the week is dated and posted. Evidence: The only menu posted was for the week of June 10th, 2019.

Plan of Correction: I will post the current weeks menu.

Standard #: 22VAC40-111-100-A
Description: Based on a review of 5 children files, the provider failed to obtain documentation of physical examination by or under the direction of a physician by the 30th day of attendance. Evidence: Child #3, #4 and #5 do not have documentation of physical by the 30th day of attendance.

Plan of Correction: I will get from parents and put in file.

Standard #: 22VAC40-111-120-B
Description: Based on review of 3 of 3 caregiver's records, the provider failed to ensure that caregiver's records are up to date. Evidence: Caregiver #3 file is missing documentation of date of hire; name address and phone number of person to be notified in an emergency and two reference checks.

Plan of Correction: I will get these and put in file.

Standard #: 22VAC40-111-60-A
Description: Based on review of 5 children's files, the provider failed to maintain up to date records for each child enrolled. Evidence: 1) Child #2 and #3's files do not have documentation of authorization for emergency pick up. 2) Child #3's file does not have documentation of the first day of attendance. 3) Child #4's files does not have documentation of medical insurance policy number.

Plan of Correction: I will get information from parents.

Standard #: 22VAC40-111-90-A
Description: Based on a review of 5 children's files, the family day home provider failed to obtain documentation of immunization on the first day of attendance. Evidence: Child #2, #3, #4 and #5 do not have proof of immunization on the first day of attendance.

Plan of Correction: I will try to get information from parents.

Standard #: 22VAC40-111-200-A
Description: Based on review of 3 of 3 caregiver's records, the provider failed to ensure that the assistant received orientation by the end of their first week of assuming job responsibilities. Evidence: Caregiver #3's file does not show documentation of orientation by the end of their first week of assuming job responsibilities.

Plan of Correction: I will do orientation and document and place in file.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the family day home provider failed to ensure that areas of the family day home outside will be maintained in a clean, safe and operable condition. Evidence: 1) The grass need to be cut so that children can run and play in backyard with good visibility. 2) The dead tree needs to be removed as it could fall on child playing in yard.

Plan of Correction: I will have grass cut and tree removed.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that poisonous substances will be stored in areas's inaccessible to children. Evidence: Torch lighter fluid stored on back porch.
