Description: Based on a review of children's medications, it was determined that the provider has administered one medication without current Medication Administration Training (MAT.) Evidence: The provider routinely administers one daily prescription medication to child B. The provider's MAT certificate expired on 4/23/19.

Plan of Correction: I have already completed the online renewal for my MAT certificate. I am scheduled to attend the in class test next week. The child who receives the medication will not be here next week.

Description: Based on a review of children's topical ointment products, it was determined that one item was not labeled as required. Evidence: One child's topical ointment product was not labeled with his name.

Plan of Correction: The name has worn off. I will label it again today.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
