Technical Assistance:
Discussed the following with the Provider: - The license renewal application must be sent to the Fairfax office prior to the expiration of the license, which is 7/29/19. - Timeframe for obtaining additional physical examinations for children - Reminder that caregiver #1 needs to obtain a TB test/screening before 7/29/19

Comments:
An unannounced renewal inspection was completed today. The physical plant, 5 caregiver/household member files, 4 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected between 12:40am-1:40pm. There were 4 children, totaling 12 points, under the supervision of the Provider. Today, the children enjoyed water play outside, learned numbers and shapes, colored pictures and cleaned up for naptime. The interactions between the children and Provider was seen to be positive and constructive in nature. The children bring their lunches to the FDH each day they are in care. The Provider is not certified in MAT (medication administration training). There are no children with any medication needs in care. Standard #: 22VAC40-111-60-B
Description: Based on file review, 2 out of 4 children's records do not have all required information. Evidence 1. Children #2 and #3 do not have a work address listed for the child's father.

Plan of Correction: The Provider will get this from the parents.

Standard #: 22VAC40-111-690-D
Description: Based on observations, the diapering surface did not meet the requirement of being non-absorbent. Evidence 1. The diaper mat has a cloth covering over it, which prevents it from being non-absorbent.

Plan of Correction: This cover will be removed.
