Technical Assistance:
Reminder to submit the license renewal application and fee to the Fairfax office prior to the expiration of the license, August 7, 2019.

Comments:
An unannounced renewal inspection was completed today. The physical plant, 5 caregiver/household member files, 3 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today between 12:50pm-2:30pm. There were 3 children, totaling 6 points, under the supervision of the Provider. Today, the children read books, colored pictures, enjoyed free play outside, ate lunch and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served egg and cheese with bread, corn, watermelon and milk. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs at this time. Description: Based on file review, 1 out of 3 children's records does not have all required information. Evidence 1. Child #1 does not have a work address for the mother.

Plan of Correction: The Provider will ask the parent to add this to the record.

Standard #: 22VAC40-111-70-A
Description: Based on file review, 2 out of 3 children's records does not have documentation that parents were given all required paperwork before their child's first day of attendance. Evidence 1. Child #1 does not have a signed "Provisions to the Emergency Preparedness Plan" or "Medication Policy" form on file. 2. Child #2 does not have a signed "Medication Policy" form on file..

Plan of Correction: The Provider will give these forms to the parents to sign.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 3 children's records does not have additional immunizations every six months up to the age of 2 years. Evidence 1. Child #2 is 27 months old. The most recent immunization updates were completed when the child was 16 months old.

Plan of Correction: The Provider will ask the parent for this.

Standard #: 22VAC40-111-210-A
Description: Based on file review, 2 caregivers do not have documentation of having received a minimum of 16 hours of annual training. Evidence 1. Caregiver #1 has documentation of having received 8 hours of training within the last year. 2. Caregiver #2 has documentation of having received 6 hours of training within the last year.

Plan of Correction: The Provider will make sure both caregivers complete this before next July.

Standard #: 22VAC40-111-240-A
Description: Based on observations, there were areas of the FDH which were not maintained in a safe and operable condition. Evidence 1. In the background, there are sections of the perimeter fence that are broken or detached from the base.

Plan of Correction: This will be fixed.
