Technical Assistance:
Clear documentation of emergency contact was discussed. Technical assistance was given regarding restroom supplies for enrolled children.

Comments:
An Unannounced Monitoring Inspection was conducted on July 31, 2019 from approximately 9:35 am until 11:45 am. There was one child in care with one provider, totaling 2 points. The sample size of records reviewed included three children's records and one provider record. The results of the inspection were reviewed with the provider during the exit interview.

Standard #: 22VAC40-111-240-A
Description: Based on observation and provider interviews, the provider did not ensure areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin.

Evidence: In the outdoor play area, there is a wooden deck and there are areas on the stairs where the paint is peeling and the wood is splintered. The deck was accessible to the child in care as they were observed playing outside near the deck.

Plan of Correction: The provider will place fence panels in front of the deck stairs to prevent access.
