Technical Assistance:
Please vary the time of day you do escape drills- five of the last six have been done in the later morning. Please make sure all adults have completed the new sworn disclosure statements on our public web site.

An unannounced monitoring inspection was conducted today with the provider, who was present with an assistant and 8 children for a total of 17 points. The children were outside playing at the beginning of the inspection. No child had medication on site and the provider reported that there have been no injuries/accidents requiring documentation. Six children and three adult files were reviewed. Arrival time: 10:40 am Departure time: 11:55 am

Standard #: 22VAC40-111-780-B
Description: Based on a review of six children's files and an interview with the provider, it was determined that the provider had not obtained annual review of the parents emergency contacts for three children ( A through C). Evidence: Child A and B enrolled in 2016 and child C enrolled on 4/30/18 and the parents have not reviewed and updated the emergency contacts annually.

Plan of Correction: We will have all parents do an annual review and keep documentation.

Standard #: 22VAC40-111-120-B
Description: Based on a review of files, it was determined that there were no references on file for Assistant A.

Plan of Correction: I will get the references.

Standard #: 22VAC40-111-60-B
Description: Based on a review of six children's files and an interview with the provider, it was determined that one child's record was missing required components. Evidence: Child C enrolled on 4/30/18 and is missing an address for one of the emergency contacts, physician name and contact information and insurance policy number.

Plan of Correction: I will get this information.

Standard #: 22VAC40-111-440-B
Description: Based on observation, it was determined that the children are using the deck which presents an entrapment hazard. Evidence: Some of the vertical slats on the deck is large enough ( 4 inches or greater) to entrap a child.

Plan of Correction: We will add mesh or some type of material to the deck to prevent an entrapment hazard. Until the mesh is added no child will be on the back deck.

Standard #: 22VAC40-191-60-B
Description: Based on a review of three adult files, it was determined that one file did not have a sworn disclosure statement on file prior to employment. Evidence: Assistant A's first day of employment was today, 7/31/19 and there was no sworn disclosure statement on file prior to employment.

Plan of Correction: Assistant A will sign the form today.
