Comments:
An unannounced monitoring inspection was conducted today from 1:20pm-2:30pm. There were 6 children (16 points) directly supervised by an assistant caregiver. The child care area, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were three medications on site today. Children were observed napping in cribs, infant seats and infant swings. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-700-A
Description: Based on medication review and provider statement, the provider accepted medication to administer without meeting all requirements for administration of medication. Evidence: Child 7 has three medications at the family day home on the date of inspection. There was not a written authorization from a parent and/or physician present for the administration of any of the three medications.

Plan of Correction: I talked to the mom and she said that she will send me what I need form the doctor.

Standard #: 22VAC40-111-710
Description: Based on a review of medication, the family day home agreed to administer medication that was not maintained in the original, labeled container. Evidence: Child 7 has an epinephrine auto injector that was not maintained in the original container with the prescription label attached.

Plan of Correction: I put it in the right spot.

Standard #: 22VAC40-111-730-B
Description: Based on observation, the family day home refrigerated medication when not needed. Evidence: Emergency medication for Child 7 was stored in the refrigerator. The medication is labeled: "DO NOT REFRIGERATE".

Plan of Correction: I moved it from the fridge in the same day.

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's full name and the date. Evidence: Two bottles were observed without a label indicating the bottle's owner or date.

Plan of Correction: I labeled all the bottles in the same day.

Standard #: 22VAC40-111-60-B
Description: Based on record review, two of five children's records reviewed were incomplete. Evidence: 1. The record for Child 2 did not contain a health insurance policy number. 2. The record for Child 5 did not contain information for one of two required emergency contacts or a written authorization for emergency medical care.

Plan of Correction: I talked to the parents and they said they will provide the infor as soon as possible.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized according to the requirements prior to the child's first day of attendance. Evidence: Child 5's record did not contain a record of immunizations.

Plan of Correction: I talked to the parents and the provided me with the paper work.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous materials were not stored in a place inaccessible to children. Evidence: Air freshener solid-gel was observed on the tank of the toilet in the bathroom used by children. The product is labeled with the statement "Keep out of reach of children".

Plan of Correction: I removed it on the same day.

Standard #: 22VAC40-111-320-I
Description: Based on observation, the family day home had an obvious fire hazard. Evidence: There was no smoke detector on the lower level including the child care area of the family day home.

Plan of Correction: I bought it in the weekend.

Standard #: 22VAC40-111-510-A
Description: Base on observation and provider statement, children were not provided with an individual crib, cot, rest mat, or bed for resting or napping. Evidence: During the inspection six children were observed sleeping. Three children were observed sleeping in cribs and three children were observed sleeping in infant seats and swings.

Plan of Correction: We have everything so we changed the sleeping way for all the kids to the correct way.
