Comments:
The licensing inspector conducted an unannounced monitoring inspection on Monday, July 22, 2019 from approximately 9:30am to 10:55am to determine the provider's compliance with licensing standards at the family day home. During the inspection, there were 9 children present totaling 18 points in the care of two caregivers. Upon the inspector's arrival, the licensing inspector observed some of the children finishing up breakfast and others in the main play area of the family day home. The licensing inspector observed the children engaged in play with their peers, coloring, singing songs, and interacting with the caregivers. The menu was posted and today's breakfast consisted of oatmeal/cereal, apple sauce, and milk. The licensing inspector interacted with the children when appropriate throughout the inspection. The caregivers were observed having positive interactions with the children in care. The required postings to include the license, emergency contacts, and recent inspection summary were inspected and found to be in compliance. The first aid kit and emergency supplies, designated childcare areas, diapering station, the bathroom, and the kitchen were inspected. During the inspection, five children's records, two caregivers' records, and one household member's records were reviewed for compliance of the licensing standards. Transportation is not provided and medication not administered at the family day home at this time. An emergency evacuation drill was documented and conducted on July 22, 2019 and the last documented shelter-in-place drill was documented on January 16, 2019. Standard #: 22VAC40-111-830-C
Description: Based on a review of documentation and interview on 07/22/2019, the provider did not ensure that the emergency evacuation drills were documented. Evidence: 1) During the inspection, the licensing inspector reviewed documentation of the emergency evacuation and shelter-in-place drills. The most recent emergency evacuation drill documented was on 04/11/2019. 2) During interview, Caregiver #1 and Caregiver #2 reported the emergency evacuation drills for May 2019 and June 2019 were conducted, but not written down.

Plan of Correction: The providers and children conducted an emergency evacuation drill for July 2019 while the inspector was on site. Drills will be documented in the future.

Standard #: 22VAC40-111-100-A
Description: Based on a review of five children's records on 07/22/2019, the provider did not ensure that one child obtained documentation of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance. Evidence: 1) The record for Child #4 (DOE: 03/18/2019) did not contain documentation of a physical examination.

Plan of Correction: The provider will obtain the physical examination from the child's parent by next Friday, August 2, 2019.

Standard #: 22VAC40-111-60-B
Description: Based on a review of documentation on 07/22/2019, the provider did not maintain a complete record at the family day home for two out of five children's records reviewed. EVIDENCE: 1) The record for Child #4 (DOE: 03/18/2019) did not have documentation of the name and policy number of the child's medical insurance, and proof of the child's identity and age. 2) The record for Child #5 (DOE: 07/10/2019) did not have documentation of the following information; emergency contact information including name address and phone number of the child's physician and two emergency contacts, information on allergies, name and policy number of the child's medical insurance, names of other persons authorized to pick up, chronic physical problems, pertinent developmental information, parents signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70, proof of the child's identity and age and written authorization for emergency medical care.

Plan of Correction: The provider will get the information from parents by this Friday, July 26, 2019.

Standard #: 22VAC40-111-90-A
Description: Based on a review of five children's records and interview on 07/22/2019, the provider did not obtain documentation that one child had been adequately immunized before the child attended the family day home. Evidence: 1) The record for Child #4 (DOE: 03/18/2019) did not contain an immunization record. 2) During interview, Caregiver #2 reported the immunization record for Child #4 was not on file at the family day home.

Plan of Correction: The provider will obtain the immunization record from the child's parent by next Friday, August 2, 2019.
