Standard #: 22VAC40-111-90-C Description: Based on child record review, updated immunizations were not obtained. Evidence: Child #3 is under the age of 2 years, last documented immunizations is dated 12/5/18. Child #5 is under the age of 2 years, last documented immunizations is dated 10/2018. Plan of Correction: I will request the updated immunizations from both of these parents.
Standard #: 22VAC40-111-240-A Description: Based on observation of the outside play area, one area of the outside was not maintained in a safe condition. Evidence: One of the fence planks located behind the swing set is not attached to the fencing, and presents a hazard to the children in care. Plan of Correction: We will repair this one plank before the children are permitted outside to play.
