Comments:
An unannounced monitoring inspection was conducted today from 3:00pm-4:15pm. There were 10 children (30 points) directly supervised by 3 staff. The home, 1 staff record, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and eating snack. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain complete children's files. Evidence: 1) Child 1 is missing start date and insurance policy number. 2) Child 2 is missing start date, address for second emergency contact, and insurance policy number.

Plan of Correction: We will have parents complete the information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider did not provide to parents in writing before the child's first day of attendance all required information. Evidence: 1) Child 1is missing operating hours, address of relocation for evacuation, holiday schedule, fees, payment schedule, communicable disease policy, discipline policies, termination policy, and sick policy. 2) Child 2 is missing operating hours, address of relocation for evacuation, holiday schedule, fees, payment schedule, communicable disease policy, discipline policies, termination policy, sick policy, liability insurance, medication policy, and emergency preparedness plan.

Plan of Correction: We will get all the missing information to the parents.

Standard #: 22VAC40-111-200-A
Description: Based on record review, the provider did not complete orientation for the assistant by the end of their first week of assuming job responsibilities. Evidence: The provider does not have documentation of completing an orientation with Staff 1.

Plan of Correction: I will make sure to complete the orientation documentation with my new staff.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the staff member did not have a central registry finding within 30 days of employment or service. Evidence: Staff 1 does not have a central registry finding on file. (Staff 1, DOH: 6/21/2019)

Plan of Correction: We have sent out the background check. You will follow up.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and interview, all staff did not submit to fingerprinting and have satisfactory results on file. Evidence: Staff 1 does not have a satisfactory fingerprint based criminal history backround check result on file. (Staff 1, DOH:6/21/2019)

Plan of Correction: My staff has an appointment on Monday, August 5, 2019.
