Comments:
An unannounced renewal inspection was conducted today from 11:00am-1:00pm. There were 3 children (6 points) directly supervised by the provider. The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, coloring, singing, and eating lunch. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: Child#1's record is missing a written authorization for emergency medical care. Child#3's record is missing the father's work address.

Plan of Correction: We will complete the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before each child's first day of attendance. Evidence: Child#1's record is missing the provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: We will print the form and have it reviewed and signed by the parents.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider did not obtain documentation of additional immunizations once every six months for a child under the age of two years. Evidence: Child#2's record contains an immunization record that was last updated on 6/1/2018.

Plan of Correction: We will ask the parents to bring a new immunization record.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all unused electrical outlets in the family day home had child-resistant protective covers. Evidence: In the lower level of the programming area behind a toy bench an unused electrical outlet did not have a child-resistant protective cover. In the kitchen area where the children eat lunch, two outlets were observed without a protective cover on top of the kitchen counter. In the napping room, 2 outlets were observed without protective covers next to the room's closet.

Plan of Correction: Corrected during the inspection.
