An unannounced monitoring inspection was conducted today from 1:15pm-2:20pm. There were 3 children (9 points) directly supervised by the provider. The home, 1 staff record, 3 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, singing, dancing, and being bottle fed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: Child#1's record is missing the address for one of two required emergency contacts. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain a current report of Tuberculosis screening/test for each caregiver and adult household member every two years from the date of the last screening. Evidence: Staff#1's record contains a tuberculosis (TB) screening/test that was last updated on 7/26/2017. The records of household members #1, #2 and #4 contain a TB screening/test that was last updated on 7/26/2017. Plan of Correction: I will make an appointment to have the records updated.
