Technical Assistance:
Discussed with provider requirements for staff files for assistant currently on leave.

Comments:
An unannounced renewal inspection was conducted today from 10:30am-1:00pm. There were a total 4 children (12 points) directly supervised by the provider. The home, 1 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site, the provider is currently certified to administer medications. Children were observed playing freely, napping, and being held while bottle feeding. Diapering procedures were also observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-70-A
Description: Based on record review, one of five children's records reviewed did not contain documentation that the parents have been informed of the provider's emergency plan. Evidence: Child 3's record did not contain documentation that the parents have been informed of the provider's emergency plan.

Plan of Correction: I will give the parents a copy of the plan.

Standard #: 22VAC40-111-90-A
Description: Based on record review, one of five children's records did not contain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: The record for Child 3's record did not contain a record of immunization.

Plan of Correction: I have asked the parents for this document already. The parents are bringing it Friday.

Standard #: 22VAC40-111-250-C
Description: Based on observation, strangulation hazards were present in occupied cribs. Evidence: The sheet used as a mattress covering is designed for cot use. Each corner of the mattress is affixed with a looped elastic band which is looped loosely over two of the corners. The excess material from the sheet is attached to the bottom of the mattress with masking tape. The elastic loops present a head entrapment and strangulation hazard.

Plan of Correction: I will purchase a new mattress and sheet today.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom did not contain all required supplies. Evidence: The bathroom did not contain paper towels.

Plan of Correction: I will replace the paper towels.

Standard #: 22VAC40-111-520-C
Description: Based on observation, the provider did not have mattresses that fit snugly next to the crib so that no more than two fingers can be inserted between the mattress and the crib. Evidence: The mattress observed in the occupied crib was 1.5 inches from the crib frame.

Plan of Correction: I will go to Walmart today and by a mattress.

Standard #: 22VAC40-111-540-A
Description: Based on observation, one child was observed in an infant seat without the protective belt fastened securely. Evidence: Child 3 was observed in an infant seat without the strap fastened.

Plan of Correction: I will strap the child in the future.

Standard #: 22VAC40-111-560-A
Description: Based on observation, the provider did not provide direct care and supervision of each child at all times including being near enough to intervene if needed. Evidence: Child 3, an infant, was placed on a large pillow located on a couch. The provider was not within arm's reach of the couch at all times to intervene if the child rolled off the pillow.

Plan of Correction: I will remain within reach of the child if she is on the couch.
