Technical Assistance:
Discussed the location of the paper towels in the bathroom used by children in care and methods to ensure they are accessible to those that can developmentally wash hands independently.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 8 children (23pts) were present today with Provider and 1 assistant. When the Inspector arrived, the children were free playing in the programming area. Staff observed engaging with one child who was adjusting to being new in care. Walk through of the entire home (all three levels), background play area and food preparation was made. The transition from audio book reading to lunch was observed. Staff ensured hand were washed. The children were observed sitting at the table and two high chairs. Lunch today was lentil soup, grilled cheese, carrots and grapes with milk to drink. Ample supply for second servings. No medications on site today and no topical ointments. A total of 5 children's records, Provider and 1 assistant record and all household members records were reviewed. This inspection began at approximately 10am ended at12:20pm. Thank you for your cooperation today. Standard #: 22VAC40-111-90-A
Description: Based on child record review, immunizations were not obtained prior to attending the family day home. Evidence: Child #1 began care on 09/06/2018. Documentation of immunizations are dated 10/09/2018.

Plan of Correction: Provider will ensure that immunizations are obtained prior to a child's start date.

Standard #: 22VAC40-111-520-C
Description: Based on observation the crib area, one mattress maintained that does not fit snuggly and allows for 2 inches of space between the mattress and crib. Evidence: The crib that is located on the wall near the kitchen area has a mattress that allows 2 inches of space in all four corners of the crib.

Plan of Correction: I will replace this mattress with another crib mattress that fits snuggly.

Standard #: 22VAC40-111-530-B
Description: Based on observation of the crib area, one crib did not have a tight fitting bottom cover. Evidence: The crib located on the wall near the kitchen area was occupied with an infant. The crib sheet was not tight fitting and allowed material to gather on the surface of the mattress.

Plan of Correction: I am replacing the mattress with a larger one that the crib sheets fit.
