Technical Assistance:
Inspectors and provider discussed the following items: water temperature substitute provider record submission of renewal application within 30 days pets

Comments:
An unannounced renewal inspection was conducted today, August 7, 2019 from 10:35 am to 1:15 pm. There were 9 children (29 points) directly supervised by 3 caregivers. At the start of inspection, the provider was not home and a substitute provider was present. The provider arrived at 10:55 am. Children were observed playing outside, napping, playing with blocks, listening to music, and playing doctor. Infants were observed crawling freely and playing with teething toys. The home, 3 staff records, 4 children's records, 1 household member's record, evacuation drills, emergency supplies and policies were inspected. There was a supply of books, toys, and materials for children. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observation, diaper ointment was kept within reach of children. Evidence: Diaper ointment was stored in a child's diaper bag. The bag was stored on a lower shelf of the changing table in the baby area accessible to children.

Plan of Correction: The diaper bag will be moved to a higher level.

Standard #: 22VAC40-111-890
Description: Based observation, the provider failed to label food brought from home with the child's name. Evidence: Two opened containers of formula were not labeled with the children's names.

Plan of Correction: We will label the formulas.

Standard #: 22VAC40-111-920
Description: Based on observation and record review, the provider failed to post a current menu for the meals and snacks provided by the family day home. Evidence: No menu was posted.

Plan of Correction: I will complete a menu today.

Standard #: 22VAC40-111-80-C
Description: Based on four records reviewed, the provider failed to document viewing each child's original proof of identification. Evidence: Child 1, 2, 3, and 4 did not have documentation that original proof of identification was viewed for each child.

Plan of Correction: I saw all the originals. I will sign and date the copies to confirm I saw the original.

Standard #: 22VAC40-111-90-A
Description: Based on four records reviewed, the provider failed to obtain documentation that all children have been adequately immunized before the child's first date of attendance. Evidence: 1) Child 1 does not have documentation of a being adequately immunized. (Child 1, DOE 7/22/2019) 2) Child 4 does not have documentation of being adequately immunized. (Child 2, DOE 7/22/2019)

Plan of Correction: Parents are aware and we have asked them for the records.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to maintain areas and equipment inside and outside of the home in a safe condition. Evidence: 1) On the outdoor playground, the right bottom wood leg of the playground is beginning to splinter. There are splinters of wood on the ground which causes a potential safety hazard to children. 2) A pizza cutter, potato peeler and apple slicer were stored in a lower unlocked drawer accessible to children causing a potential safety hazard.

Plan of Correction: I have made an appointment to fix the leg of the playground At the time of inspection, the pizza cutter, potato peeler and apple slicer were moved into an upper cabinet out of the reach of children.
