Comments:
An unannounced monitoring inspection was conducted today, 8/1/19. There were 9 children present (20 points) with 3 caregivers. Children were observed putting on shoes, having sunscreen applied, playing outside on the playground with balls, on the swings and the slide, and running together. Lunch was observed cooking (chicken, rice, beans, and fruit) Reviewed 3 staff files, 1 resident file, 2 children's files, attendance, menus, emergency evacuation log, cribs, and physical plant. Standard #: 22VAC40-111-60-B
Description: Based on record review and interview with Provider, children's records were incomplete. Evidence: Child#1 and Child#2 were missing written authorization if the child is taken off the premises of the family day home. Provider stated they take the children on walks around the neighborhood.

Plan of Correction: I will have parents sign permission
