Technical Assistance:
Discussed Emergency Preparedness and Response plans and outdoor play equipment.

Comments:
An unannounced monitoring inspection was conducted today from 10:00am-1:00pm. There were 12 children (27 points) supervised by 2 direct care staff. The home, 2 staff records, 6 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed having outdoor water play, playing with push toys, playing in the sandbox, eating snack and lunch from home, and playing freely. Handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-770
Description: Based on observation, a working portable battery-operated weather band radio was not available to caregivers at all times. Evidence: An available battery-operated radio did not turn on with batteries provided.

Plan of Correction: I will get batteries.

Standard #: 22VAC40-111-830-A
Description: Based on review of records and interview with staff, emergency evacuation procedures were not practiced monthly. Evidence: A fire drill was not documented during the months of January 2019 and May 2019.

Plan of Correction: I will make sure to do the fire drills every month.

Standard #: 22VAC40-111-120-B
Description: Based on record review, 1 of 2 staff records did not contain documentation of two or more references checks as to character and reputation as well as competency, and verification of age. Evidence: 1. Staff 2's record did not include a signature of the person obtaining the reference or date received. 2. Staff 2's record did not contain verification of Staff 2's age.

Plan of Correction: I will sign and date reference page. I will get a copy of driver's license.

Description: Based on review of children's records, 6 of 6 children's records did not contain documentation of all required information. Evidence: 1. Child #1's record did not contain documentation of the child's first date of attendance. 2. Child #2's record did not contain documentation of a complete address for the second emergency contact person. 3. Child #3's record did not contain documentation of proof of identification, and an annual review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented parent review of the child's emergency contact information was dated 2/23/2018. 4. Child #1, 2, 3, 4, 5, and 6's records did not contain documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70, and written authorization if a caregiver is to administer prescription or nonprescription medication to the child as required by 22 VAC 40-111-700 A 2.

Plan of Correction: I will get necessary documentation.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored in areas inaccessible to children. Evidence: 1. Seven containers of shaving foam were located in the bottom of changing table located in the bathroom. 2. A container of hairspray, alcohol, and lysol were located above the sink in the bathroom.

Plan of Correction: I will move the items.
