Comments:
An unannounced renewal inspection was conducted today from 10:15am-12:00pm. There were 7 children (20 points) supervised by 3 direct care staff. The home, 4 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside, coloring, and eating lunch. Lunch served was chicken nuggets, vegetable soup, strawberries, and milk. Diapering and handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-750-A
Description: Based on observation and record review, the provider did not obtain written parent authorization when using topical skin products. Evidence: Four children's sunscreen containers and 8 children's insect repellent containers were found on the outdoor play area, and did not have corresponding parent authorization forms.

Plan of Correction: I will print out the authorization forms and have parents sign them.

Standard #: 22VAC40-111-750-B
Description: Based on observation, the provider did not ensure that topical skin products were inaccessible to children. Evidence: 1. Three diaper cream containers were sitting on the diaper changing table. 2. A plastic container containing children's suncreen and insect repellent containers was sitting on a table in the outdoor play area.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 4 children's records did not contain documentation of all required information. Evidence: 1. Child #2's (start date: 9/4/2018) record did not contain documentation of a completed physical form signed by a physician. 2. Child #3's (start date: 7/8/2019) record did not contain documentation of either parents place of employment.

Plan of Correction: I will get the parents to bring a copy of the physical tomorrow, and will have parents tell me their place of employment this afternoon.

Standard #: 22VAC40-111-460-C
Description: Based on record review, the provider did not ensure that the dog was vaccinated for rabies as needed. Evidence: The dog's vaccination record documented a rabies vaccination that expired 7/30/2019.

Plan of Correction: I will tell the provider's daughter to take the dog to the vet.
