Technical Assistance:
Discussed cleaning supplies, the Information for Parents form, and children interactions.

Comments:
An unannounced monitoring inspection was conducted today from 2:45pm-5:00pm. There were 10 children (29 points) supervised by 2 direct care staff. The home, 1 staff records, 6 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping, watching a video, playing freely, and eating snack. Diapering and handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, the provider did not ensure that children were not allowed to eat or drink while walking, running, playing, lying down, or riding in vehicles. Evidence: 1. Child #8 was lying on a naptime mat drinking a bottle. 2. Child #7 was lying in a crib drinking a bottle.

Plan of Correction: I will read the regulations again and fix it moving forward.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider did not ensure that bottles were labeled with the child's full name and the date, if more than one infant is in care. Evidence: 2 infant bottles were not labeled with the current date.

Plan of Correction: I will check with the parents and remind them to put dates on bottles.

Standard #: 22VAC40-111-60-B
Description: Based on children's records, 3 of 6 children's records did not contain all of the required documentation. Evidence: 1. Child #2's record did not contain documentation of the medical insurance policy number. 2. Child #4's record did not contain documentation of the first date of attendance, medical insurance information, proof of identification, and a completed physical signed by a physician. 3. Child #5's record did not contain documentation of the first date of attendance, the father's place of employment, medical insurance information, and a completed physical signed by a physician.

Plan of Correction: I will get all missing information from the parents.
