Annual training hours required is 16.

Comments:
An unannounced monitoring inspection was conducted from 12:00pm until 2:45pm. There were 13 children present (31 points); however 1 child resides in the home. There were 3 caregivers present. The children were observed sitting in immobile toys, playing in the sand box and napping. The physical plant, emergency plans, emergency supplies, bottles, cribs, evacuation drill logs and menu's were inspected. Areas of non compliance are cited on the Violation Notice. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on inspection, the family day home failed to ensure the first aid kit contained the minimum supplies. Evidence: The first aid kit was missing gauze pads, tweezers, disposable gloves, 1 triangular bandage and a first aid instruction manual.

Plan of Correction: I will buy items and place in first aid kit.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the family day home failed to ensure that all bottles will be labeled with the child's full name and date if more than one infant is in care. Evidence: There were 3 bottles that were not labeled with the child's full name and the date.

Plan of Correction: I will make sure that bottles are labeled with name and date.

Standard #: 22VAC40-111-120-A
Description: Based on review of records, 2 of 3 caregivers records reviewed, the family day home failed to ensure that caregiver records were complete. Evidence: 1) Caregiver #2 and #3's records do not include documentation of 2 reference checks or information for emergency contacts.

Plan of Correction: I will make sure they get reference and I will put in caregivers records.

Standard #: 22VAC40-111-50-A
Description: Based on observation and interview with provider, the family day home failed to ensure that a written record of children's attendance will be kept each day. Evidence: The family day home did not have a record of attendance for the date of inspection - 08/07/2019.

Plan of Correction: I forgot to do attendance today, but will remember moving forward.

Standard #: 22VAC40-111-60-A
Description: Based on , 2 out of 5 children's records reviewed, the family day home failed to ensure that children's records are kept up to date for each child enrolled. Evidence: 1) Child #2's records do not have documentation of a second emergency contact or names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will get the information from parents and place in child's file.

Standard #: 22VAC40-111-280
Description: Based on observation, the family day home did not ensure that potentially poisonous substances will be stored away from areas accessible to children. Evidence: 1) Hand sanitizer was on shelf beside changing table. 2) Glass cleaner and a bleach based cleaner were on the shelf in the bathroom.

Plan of Correction: I will remove them and store them away from children.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the family day home failed to ensure that child resistant covers will be installed on all unused electrical outlets and surge protectors. Evidence: There was one outlet without a protective cover in the bathroom.

Plan of Correction: I will replace.
