63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:10am-12:20pm. There were 5 children (14 points) in the direct care and supervision of 2 caregivers. One household member's record was reviewed and one caregiver's record was reviewed. The children were observed playing in play area and engaged with caregivers. Provider left out of town for an emergency. One new caregiver has been hired since the last inspection. Areas of non-compliance are identified in this report. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider failed to obtain a current tuberculosis report for every household member every two years from the date of the first screening. Evidence: 1. Household Member A did not have a current tuberculosis screening on file. 2. The tuberculosis screening of Household Member A was last completed 05/ 2015.

Plan of Correction: I will email the doctor to try and get an appointment.

Standard #: 63.2-1721.1-B-2
Description: Based on interview and record review, the provider failed to ensure that all household members completed the fingerprinting process as required by subdivision B 2 of ? 19.2-392.02. Evidence: 1. Household Member A did not initiate the fingerprinting process. 2. Caregiver A stated Household Member A did not conduct the fingerprinting process.

Plan of Correction: She is ill and unable to leave the house.
