Technical Assistance:
Discussed the supervision point system with the Provider.

Comments:
An unannounced renewal inspection was completed today. The physical plant, 3 caregiver/household member files, 3 children's files, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 10:40-11:50am. There were 3 children, totaling 11 points, under the supervision of the Provider. Today, the children played with puzzles, listened to storytime, enjoyed free play, ate lunch and rested during nap time. The interactions between the children and caregiver was seen to be positive and constructive in nature. Today, the children were served chicken with rice, mixed vegetables, pears and milk. The Provider is not currently certified in MAT (medication administration training) and there are no children with any medication needs at this time. Standard #: 22VAC40-111-70-A
Description: Based on file review 1 out of 3 children's records does not have all required information to be given by the Provider prior to the child's first day of attendance. Evidence 1. Child #3 does not have the "Provisions to the Emergency Preparedness Plan"on file.

Plan of Correction: The Provider will give this to the parents to sign and return.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 3 children's records does not have additional immunizations every six months up to the age of 2 years. Evidence 1. Child #1 is 20 months old. The most recent immunization record on file was completed when the child was 12 months old.

Plan of Correction: The Provider will ask for this

Standard #: 22VAC40-111-340-C
Description: Based on observations, the bathroom that the children use does not have all required items in it. Evidence 1. The bathroom did not have paper towels in the bathroom.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-690-D
Description: Based on observations, the diapering surface was not non-absorbent. Evidence 1. The diaper mat had a few rips in it, which prevented it from being non-absorbent.

Plan of Correction: The Provider will replace this.
