54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Annual staff training was not reviewed during this inspection and will be reviewed at the next licensing inspection. Discussion on food preparation for meals/snacks and the requirement of supervision.

Comments:
Unannounced Monitoring inspection was conducted this date. A total of 2 children (6 pts) with Provider was present. When the Inspector arrived, the children were transitioning to the outside play space located in the garage. The area was equipped with ample toys and equipment age appropriate for the children currently in care. The home was found organized and well equipped with Montessori equipment, books, tactile activities for the children. The equipment was located within reach of the children for self selection and independent choice. Provider had taken the children outside to blow bubbles and to complete table top activities such as memory cards. Preparation for lunch was observed. Bathroom procedures were also observed with one child and Provider. No medications on site at this time. No topical ointments on site. A review of 3 children's records, all household and Provider record was completed. This inspection began at approximately 9:25am and ended at 11:15am. Standard #: 22VAC40-111-90-A
Description: Based on child record review, documentation of immunizations for one child was incomplete. Evidence: Child #2 began care on 6/24/19, the immunization documentation for Child #2 was received on 6/12/19. The immunizations were not signed and dated by the physician.

Plan of Correction: Additional immunizations were obtained on 6/25/19 which were signed and dated by physician.
