Comments:
An unannounced monitoring inspection was conducted on July 31, 2019 from approximately 9:55 am to 1:00 pm. Provider and assistant were present with eleven children for a total of twenty-four points. Five children's records were reviewed. Provider and household member records were also reviewed. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. If you have any questions, contact your licensing inspector at (757) 404-5261. The providers responses for the ?Description of Action To Be Taken? were not received as of August 13, 2019 and will not appear on this ?Violation Notice?.

Standard #: 22VAC40-111-920
Description: Based on observation, provider did not ensure when meals or snacks are provided by the family day home, the menu for the current week is posted. Evidence: The menu posted was dated "Monday 15 - Friday 19". "July 31st" was added on the space allocated for Saturday.

Description: Based on review, provider did not ensure 1 of 5 children's records has a physical examination within 30 days of child's first day of attendance. Evidence: The record for child #2 (start date December 10, 2018) did not have a physical.

Standard #: 22VAC40-111-40-A
Description: Based on observation and interview, provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: The home is licensed for nine children. There were eleven children present in care at the family day home. Provider stated two children are attending as a "trial" for a few hours two days this week and two days next week with the plan to start attending full-time in September.

Description: Based on review, provider did not ensure 2 of 5 children's records have documentation of required information. Evidence: The following was found during review of records: 1. The record for child #2 (start date May 24, 2019) did not have mother's home information, mother's employment information, emergency contact information and notification of liability insurance coverage. 2. The record for child #3 (start date December 10, 2018) did not have an address and phone number for the second emergency contact.
