An announced Initial Inspection was conducted today from 9:45am to 11:10am. There were 3 children (7 of points) with the Provider present today, within the supervision guidelines. Provider and 3 household members records were reviewed and found complete. All enrolled children?s files (4) were reviewed. Overall, good systems are in place. The home was found clean and supplied with ample toys and programming equipment appropriate for the children currently in care. Adequate space determined for the requested capacity of 7 children. Children were observed watching TV, eating a morning snack (goldfish crackers and yogurt), socializing and other organized activities. The provider is not currently MAT trained to administer medications to the children but is considering taking the training in the near future. Emergency procedures, provider contract, First Aid/CPR certification was verified. There are currently 3 cribs found on site and meet the CPSC requirements. Violations found today were reviewed with the provider and an appropriate corrective action was provided. Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information. Evidence: 1. 4 out of 4 children's records did not contain addresses for emergency contacts. 2. 4 out of 4 children's records did not contain written authorization to leave the premises. The provider stated that they go to the local playgrounds for their outside play time. Plan of Correction: I'll make a copy and request the parents to fill out the missing information today when they pick up.
