- Technical Assistance:
Discussed with the provider that parents need to completely fill out the enrollment forms and provide the required information. The provider may continue to use her own forms, but know that the VDSS sample enrollment forms contain all of the required information needed and she will just need to ensure that the parents do not leave any of the lines blank. Also discussed with the provider that one parent does not wish to sign the emergency medical authorization form. According to standard 22VAC40-111-(2)-60-B8: (the child's record shall contain) written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. The provider must obtain documentation of the parents objection and it is recommended that the parent provide a plan the provider may follow in the event of the child having a medical emergency and the parent cannot be reached.
An Unannounced Monitoring Inspection was conducted on 8/13/2019 from 11:20am to 12:45pm. There were 3 children (7 points) directly supervised by 1 caregivers. The home, 2 caregiver's records, 6 child's records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-800-B Description: Based on review, the provider failed to ensure that the emergency preparedness plan was reviewed and updated as needed on an annual basis. Evidence: Reviewed the emergency preparedness plan and found that the last documented update was on 7/18/18. Plan of Correction: Corrected at the time of the visit.
Standard #: 22VAC40-111-830-A Description: Based on review, the provider failed to practice emergency evacuation procedures on a monthly basis. Evidence: Reviewed the provider's records and found that they did not have documentation that they practiced an emergency evacuation drill in July of 2019. Plan of Correction: Will make sure I do them monthly.
Standard #: 22VAC40-111-60-B Description: Based on review, the provider failed to ensure that children's records were maintained with the required documentation. Evidence: Reviewed 6 children's records and found the following missing: 1) Child 2, 3, and 4 did not have documentation of complete parent work information (place, address and number). 2) Child 3 did not have documentation of addresses for either of their emergency contacts. 3) Child 1 and Child 4 do not have documentation of emergency medical authorization. 4) Child 2, 3, 4, 5 and 6 do not have documentation of medical insurance policy numbers. Plan of Correction: Will have parents complete within a month.
