Technical Assistance:
Discussed the following with the Provider - requirements for visiting family members - requirements for adult household members - timelines for immunization requirements of children

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 6 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:00-2:15pm. There were 6 children, totaling 12 points, under the supervision of 2 caregivers. Today, the children ate lunch, cleaned up from eating, enjoyed free play, listened to storytime, changed into their pajamas and took a nap. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children were served vegetable soup, pasta, with ground beef, bread and milk. There are currently 2 caregivers certified in MAT (medication administration training), CPR and First Aid. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-111-830-A
Description: Based on observations, the emergency evacuation drill log does not have documentation of drills being completed each month that children receive care. Evidence 1. There most recent documentation of an evacuation drill having been completed was on April 17, 2019.

Plan of Correction: These will be completed.

Standard #: 22VAC40-111-100-A
Description: Based on file review, 1 out of 5 children's records does not have a physical examination. Evidence 1. Child #3 does not have a physical on file.

Plan of Correction: I will get this from the parent.

Standard #: 22VAC40-111-180-A
Description: Based on file review, 2 out of 2 caregivers do not have additional TB test/screenings completed every two years. Evidence 1. Caregivers #1 and #2 have TB tests which were last completed in July 2017.

Plan of Correction: These will be completed.
