Comments:
An unannounced monitoring inspection was conducted today between 9:50 am and 12:15pm. There were 7 children present with 3 caregivers. The children were observed hand washing, singing and playing with floor toys and puzzles. There is an abundant supply of materials and books for the children. The physical plant, emergency supplies, children's records, caregiver records, household member records, cribs, attendance logs and evacuation drill logs. Standard #: 22VAC40-111-60-A
Description: Based on a review of 5 children's records, in 1 out of 5 records, the family day home failed to maintain up to date records for all children enrolled. Evidence: Child #3's record has not documentation for names of persons other than the custodial parents who are authorized to pick up the child,

Plan of Correction: I will get information from parents and put in child record.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the family day home failed to ensure that areas and furnishings of the family day home inside and outside shall be maintained in a clean, safe and operable condition. Evidence: In the outside play area, the children's stove has a hole on the top that could snag the skin of a child sticking their finder into it.

Plan of Correction: I will repair the hole in the outside kitchen set in play area.
