Comments:
An unannounced monitoring inspection was conducted today between 12:30pm and 3:30pm, there were 11 children present (24 points) with 2 caregivers upon inspectors arrival. The provider was over her licence capacity of 10 children. The children were observed eating lunch, singing and playing with floor toys. Lunch consisted of Chicken, rice, vegetables (mixed) apples, banana's and milk. The physical plant, attendance logs, menu's, emergency supplies, children's records, caregiver's records and cribs were inspected. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-111-100-A
Description: Based on review of 5 children's records, 3 of 5 did not have documentation of current physicals within 30 days after the first day of attendance. Evidence: Child #1, #4 and #5's records did not have documentation of a physical with 30 days after the first day of attendance.

Plan of Correction: I will get forms from parents.

Standard #: 22VAC40-111-120-B
Description: Based on a review of 3 caregivers, 1of 3 records were incomplete. Evidence: Caregiver #3's record did not have documentation of date of employment, name and address and phone umber for emergency contact, documentation of 2 references.

Standard #: 22VAC40-111-40-A
Description: Based on observation the provider failed to ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: The license for the FDH allows a maximum of 10 children. There were 11 children in attendance today.

Plan of Correction: I will not have more than 10 children.

Standard #: 22VAC40-111-60-A
Description: Based on a review of 5 children's records, 2 of 5 records were not maintained and up to date. Evidence: 1) Child #2's record did not have documentation of names of persons other than the custodial parents who are authorized to pick up the child or the liability insurance form.

Plan of Correction: Child will not be in care after 8/26/19

Standard #: 22VAC40-111-90-A
Description: Based on a review of 5 children's records, 3 of 5 record did not have documentation of immunizations by the first day of attendance. Evidence: Child #1, #3 and #4's record did not have documentation of immunizations by the first day of attendance.

Standard #: 22VAC40-111-180-A
Description: Based on review of 3 caregiver records, 2 of 3 caregiver records did not have documentation of current tuberculosis (TB) screening or test. Evidence: Caregivers #2 and #3 did not have documentation of current TB test or screening.

Plan of Correction: I will have them get current TB test or screen

Standard #: 22VAC40-111-280
Description: Based on observation the provider failed to store potentially poisonous substances in area not accessible to children. Evidence: 1) In the upstairs bathroom a bleach and water solution was on the counter accessible to children. 2) A bottle of alcohol was on the counter accessible to children.

Plan of Correction: I will move the solution and alcohol from the counter.

Standard #: 22VAC40-111-540-A
Description: Based on observation, high chairs will be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened. Evidence: 2 of the 5 high chairs occupied, did not have safety strap used and securely fastened.

Plan of Correction: I will check straps

Description: Based on a review of 3 caregivers records, 1 of 3 records did not have documentation of a complete sworn statement or affirmation prior to employment or volunteering. Evidence: Caregiver #3's record did not have documentation of a sworn statement prior to employment or volunteering.

Plan of Correction: I will have her complete a sworn statement.

Standard #: 63.2-1720.1-B-2
Description: Based on a review of 3 caregivers records , 1 of 3 did not have documentation of submission of Criminal History Records check prior to first day of employment or volunteering. Evidence: Caregiver #3's record did not have documentation of Criminal History Records check prior to date of employment or volunteering.

Plan of Correction: I will have her submit for fingerprints.
