Technical Assistance:
Had a discussion with the provider about contacting Subsidy so that the VDSS website reflects that they are a subsidy vendor.

Today an unannounced Renewal inspection took place between 9:25am and 11:15am. There were 4 children present with the provider for a total of 7 points within the supervision guidelines. The children were observed playing with play doh, bowling, doing puzzles and being read to. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-920
Description: Based on record review, the menu is not dated. Evidence: There is a food menu posted on the side of the provider's refrigerator. At the stop of the menu it says "2019" however the month and dates are not filled in.

Plan of Correction: My menu is also posted online. I'll add the dates to the menu posted.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1, Child #2 and Child #3's file did not contain a phone number for their second emergency contact.

Plan of Correction: I have talked to mom about this contact and she said the person does not have a phone.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation of immunizations before each child may attend. Evidence: Child #2 file did not contain documentation of immunizations. This child was observed in care today.

Plan of Correction: Mom was contacted during the inspection. Mom is going to contact the doctor's office to have the most recent immunizations faxed to the provider.
