Comments:
Today an unannounced Monitoring inspection took place between 11:10am and 1:05pm. There were 9 children present with the provider and 1 assistant for a total of 17 points within the supervision guidelines. The children were observed playing freely, washing their hands, eating lunch (macaroni and cheese with broccoli and watermelon) and napping. One child does not take a nap and was observed practicing reading with the caregivers. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #5's file did not contain addresses for both emergency contacts, the families medical insurance policy number, or documentation of a signed insurance declaration.

Plan of Correction: This child is currently on vacation and their last day is at the end of the month. I don't know if they'll be back. If they come back we'll have the parents fill this in.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a subsequent Tuberculosis (TB) screening for each household member. Evidence: Household member #1's TB screening expired 3/22/2019.

Plan of Correction: They are currently seeking treatment for a risk. Their treatment will be finished in November.

Standard #: 22VAC40-111-690-L
Description: Based on observation, toilet chairs were not emptied promptly, cleaned and sanitized after each use. Evidence: At approximately 11:25am two toilet chairs were observed in the children's bathroom with urine sitting in them. Both chairs were emptied at approximately 12:15pm, rinsed with water and returned to the floor.

Plan of Correction: I'll start using the same cleaner that we use on our changing table.
