Comments:
An unannounced monitoring inspection was conducted from 12:00pm-2:00pm. There were 10 children (24 points) in the direct care and supervision of 3 caregivers. The Provider had stepped out, however came within 10 minutes of the start of the inspection. A sample size of 4 children's records, 3 caregiver records, emergency materials, evacuation drills, and the day care space were reviewed. The children were observed being fed a bottle and taking a nap. If you have any questions, please contact me at shahana.green@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-530-A
Description: Based on observation, 1 child did not have linens consisting of a top and a bottom cover. Evidence: 1. Child #6 was laying on a cot that did not have a bottom cover.

Plan of Correction: We will put the bottom cover on.

Standard #: 22VAC40-111-530-F
Description: Based on observation, it was determined that soft bedding was placed inside cribs occupied by infants. Evidence: 1. Child #1 had a soft blanket placed loosely around them while sleeping in a crib. 2. Child #4 had a soft blanket placed loosely around them while sleeping in a crib.

Plan of Correction: We will use an appropiate sleep sack or swaddle for the infants when they sleep.
