Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, August 13, 2019. The on-site inspection was from 10:40a.m. to 12:45p.m. The provider was caring for two children (4 points). There were a total of seven non-resident children enrolled in the program. The exception to compliance was noted on the violation notice and the risk rating was listed on the supplemental page. There were five children's records reviewed along with the record of the provider. There were no medications on-site or currently being administered, which is the policy of the provider. Some of the items reviewed and observed included: emergency supplies, documentation of evacuation, shelter-in-place, and lockdown drills, first aid kit, updated emergency preparedness plan, warm water temperature, refrigerator and freezer temperatures, required posted information, updated rabies vaccinations for one dog, arrival procedures, choice time, infant nap time, and TV time. Description: Based on review of five children's records and confirmation from the provider, the provider failed to have the parent of three children review their emergency contact information and obtain the parent's signed acknowledgment of the review. Evidence: 1. The first day of attendance for Child #1 was 05/21/2018. That was the last date the parent had reviewed the emergency contact information. 2. The first day of attendance for Child #2 was 05/01/2015. The most recent date the parent had reviewed the emergency contact information was 03/07/2018. 3. The first day of attendance for Child #4 was 02/16/2011. The most recent date the parent had reviewed the emergency contact information was 08/01/2018.

Plan of Correction: I will write a reminder on my information board so I won't forget to have the parents update the information every year.
