Comments:

The licensing inspector conducted an unannounced renewal inspection on 8/15/2019 from 9:45am to 12:05pm. The infants were observed having floor play, tummy time and eating. Today, there were two children, totaling 8 points, present with two caregivers. The caregivers were observed interacting with the children. Interviews were held with the caregivers throughout the inspection. Additionally, when appropriate the inspector interacted with children. The menu was posted. The provider does not administer medication at this time. The provider does provide transportation. The provider has one pet. Five children?s records, three caregiver records, and one household member record was reviewed during the inspection. Last emergency drill: 7/25/2019 Last shelter-in-place drill: 2/15/2019 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the provider. A complete renewal application and fee must be received by the Department prior to expiration of the license on September 29, 2019 or the license cannot be renewed. Standard #:	22VAC40-111-180-A
Description:	Based on observation, the provider did not ensure that two of four caregiver/household member records had documentation of a current report of Tuberculosis Screening form every two years from the date of the first screening. Evidence: 1. The record of caregiver #1 (start date: 2/04/1996) had documentation of a tuberculosis screening dated 1/30/2017. 2. The record of household member #4 (start date: 2/04/1996) had documentation of a tuberculosis screening dated 2/28/2017.
Plan of Correction:	We will get the tuberculosis screenings no later than Tuesday.
