Technical Assistance:
Discussed with provider that documentation confirming the family day home's status as an LLC may be submitted with renewal paperwork to include the LLC name on the license. Reminder: A minimum of two shelter in place drill must be conducted annually. A minimum of sixteen hours of training for all staff is required annually.

Comments:
An unannounced monitoring inspection was conducted today from 11:00am-12:15pm. There were 10 children (31 points) directly supervised by the provider and assistant. The child care area, 3 staff records, 5 children?s records, 2 household member?s records, pet vaccination records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing with care givers building blocks, dancing, playing instruments, and blowing bubbles. Hand washing practices were observed for children prior to eating lunch brought from home. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider's emergency plan was not reviewed at least annually. Evidence: The most recent documented review of the emergency preparedness plan was conducted on May16, 2018.

Plan of Correction: I will review and update the emergency plan as needed.

Standard #: 22VAC40-111-250-A
Description: Based on observation, hanging ribbon was accessible to children under five. Evidence: One baby gate was observed to be tied into an open position by ribbon stretching from the gate to the base of the stairs. The ribbon creates a possible strangulation hazard to children.

Plan of Correction: I will remove the ribbon.

Description: Based on record review, wood burning stoves and fireplaces and associated chimneys were not inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence: The most recent inspection was conducted May 11, 2018.

Plan of Correction: I will schedule a new inspection.
