Technical Assistance:
Technical assistance was provided regarding children's files and the Emergency Preparedness Plan.

Comments:
An unannounced renewal inspection was conducted between the hours of 8:55am through 12:00pm. There were a total of 5 children present ages 4 years to 11 years (6 points) with the Provider. A total of 5 children?s files, the provider's file, and 1 household member?s files were reviewed. The children were engaged in the following: playing video games and dancing. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. The provider advised she does not administer medication and no medication is on site today. There was an abundant supply of age appropriate materials and toys. Standard #: 22VAC40-111-830-C
Description: Based on review of documentation and Provider interview, the emergency drill log was not complete. Evidence: 1. There was no documentation of a fire drill for January 2019 and May 2019.

Plan of Correction: There was a drill done each of these months, but in the future I will document all drills.

Standard #: 22VAC40-111-920
Description: Based on observation of the Family Day Home and Provider interview, there was not a current menu posted or provided to parents. Evidence: 1. The menu posted was dated May 2019. 2. The Provider stated a current menu is not posted and was not provided to parents.

Plan of Correction: For the Summer program, I have verbally told parents what is being served each day. In the future, all menus will be posted.

Standard #: 22VAC40-111-60-B
Description: Based on review of 5 children's records, 3 children's records were incomplete. Evidence: 1. Child #1 was missing 1 of 2 required emergency contacts, medical insurance policy number, documentation of the annual review of the child's record. Child #1's record was last reviewed 1/18/17. 2. Child #4 was missing a complete address for 1 emergency contact. Child #4 was also missing documentation of the annual review of the child's record. Child #4's record was last reviewed 2/6/14. 3. Child #5 was missing phone numbers for 2 emergency contacts, medical insurance policy number, and results of a health exam within 30 days of first day of attendance. Child #5's first day of attendance was May 2019.

Plan of Correction: That will be completed by the parents upon arrival today.

Standard #: 22VAC40-111-180-A
Description: Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years. Evidence: 1. Adult Household Member #1's most recent TB screening is dated 4/4/17.

Plan of Correction: I will make the appointment as soon as possible.
