Comments:
An unannounced renewal inspection was conducted today from 11:30am-12:30pm. There were 7 children inclusive of the provider's children (16 points) directly supervised by the provider and assistant. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were two medications reviewed on site today. Children were observed playing freely, washing hands, and eating lunch. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-710
Description: Based on a review of medication, the family day home agreed to administer medication that was not maintained in the original labeled container with the prescription label attached. Evidence: Child 2's emergency medication was not maintained in the original container with the prescription label attached.

Plan of Correction: I will ask the parents for the labeled container.

Standard #: 22VAC40-111-70-A
Description: Based on record review, there was not documentation present to confirm that parents had been provided with all required information. Evidence: 1. Children 1-5's record do not contain a signed acknowledgement of receipt of the provider's medication policies. 2. Child 4's record does not contain a signed acknowledgement that the provider has informed the parents of the provider's liability insurance.

Plan of Correction: I will have the parents sign the forms.

Standard #: 22VAC40-111-320-B
Description: Based on observation, one outlet was observed without a protective cover. Evidence: One outlet in the child care area was observed without a protective covering.

Plan of Correction: Corrected during inspection.
