Comments:
A unannounced mandated Renewal inspection during the Provisional Licensure period was conducted today, 8/22/2019. There were 9 children (21 points) with 3 caregivers. Children were observed playing with puzzles, dancing with scarves, playing with playdoh, and playing outside. Reviewed 3 children's files, 3 staff files, 1 resident file, emergency evacuation log, menus, attendance, crib compliance, and physical plant. Violations previously cited were also reviewed for compliance. Violations of the Standards are recorded on the Violation Notice. Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child#3 (start date 7/11/18) did not have documentation of a physical within 30 days of the first day of attendance.

Plan of Correction: I will ask the parent for a copy
