Comments:
An unannounced inspection was conducted on 08/21/2019, from 1:00 p.m. to 2:50 p.m. to review licensing requirements. There were 3 children present, 21 months to 9 years old, for a total of 5 points, in care with the provider. The records sample size consisted of 3 newly enrolled children?s records and 1vendor record. Children were observed watching a children's program, resting quietly, playing with toys and dancing to music. The vendor states that no medication is administered to children in care at this time and that children are transported in a vehicle both for outings and to and from care. Requirements for transportation were reviewed with the vendor - but transportation was not observed during the inspection. Children play outdoors in the back yard with direct supervision and close proximity of the provider. The provider reports that there have been no accidents or injuries requiring first aid or medical treatment incurred by children in care since the last inspection. There are no children currently enrolled who have diagnosed food allergies. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Standard #: 22VAC40-111-100-A
Description: Based on a review of records and interview, the provider has not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Four children's records were reviewed. The records for child #1(enrolled 2/4/19), child #2 (enrolled 2/16/19) and child #4 (enrolled 2/16/19) did not contain the results of a physical examination. The provider confirmed that she did not have documentation of the children's physical exam.

Plan of Correction: The provider stated that she would request that the parents have this information prepared for the children and will pick them up from the children's doctor's office by the end of next week.
