Comments:
An unannounced inspection was conducted today from 9:00am until 11:45 am. There were 4 children (16 points) present with 3 caregivers. The children were observed sleeping, being bottle feed and playing with toys. Interactions with caregivers were positive. The physical plant, cribs, emergency plans, menu's, attendance logs, evacuation logs and emergency supplies were inspected. There were no medications on site. Violations:
Standard #: 22VAC40-111-120-A
Description: Based on a caregiver records review, in 1 of 3 caregivers records, the provider failed to ensure that caregivers records are maintained and up to date. Evidence: Caregiver # 3 does not have documentation that two or more references in caregiver record.

Plan of Correction: I will get second reference.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, in 2 of 4 records the family day home failed to ensure that records are maintained and up to date for each enrolled child. Evidence: 1) Child #2's record does not contain the name, address and telephone number of each custodial parent's place of employment. 2) Child #3's record does not contain the information for parent form or the parent agreement and emergency plan.

Plan of Correction: I will get from parents and place in children's records.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, in 1 out of 4 records the family day home failed to ensure that each child has proof of immunizations by the first day of attendance. Evidence: Child #4's record does not contain documentation of immunizations.

Plan of Correction: I will get it from parent and place in child file.

Standard #: 22VAC40-191-60-C-2
Description: Based on caregiver record review, 1of 3 records reviewed did not have a central registry finding within 30 days of employment or volunteer service. Evidence: Caregiver #3 does not have documentation of a central registry finding within 30 days of employment.

Plan of Correction: The form has been completed, but is not available. I took form to Arlington County and I will contact them to see if they still have.
