Technical Assistance:
Discussed: Supervision of infants - monitoring of each sleeping infant in one of the following ways: by placing each infant for sleep in a location where the infant is within sight and sound of a caregiver or by in-person observation of each sleeping infant at least once every 15 minutes. Requirement to have 12 evacuation drills (1 per month) in addition to 2 shelter-in-place drills. One lockdown drill is required a year for subsidy vendors.

Comments:
An unannounced monitoring inspection was conducted from 1:00 - 3:00 pm with the provider and her assistant. There were eight children in care, ranging from 11 months to ten years of age, for a total of 18 points. The children were observed napping druing today's inspection. One school age child was watching an age appropriate video on an electronic device. Four children's records were reviewed. Four caregiver/household member records were reviewed and in compliance. Required information was posted. Childcare areas and supplies were observed. The emergency drill log was reviewed and in compliance. The provider is Medication Administration Training (MAT) certified but there were no medications to review. Standard #: 22VAC40-111-60-B
Description: Based on a review of four children's records, not all records were complete. Evidence: the file for Child #2 and child #3 did not have signed documentation that parents had been given VDSS model forms "Information for Parents", "Policy on the Administration on Medication", or ?Liability Insurance Declaration" in their record. Child #3 did not have written authorization for emergency medical care.

Plan of Correction: Will obtain the missing information from the parents tonight.
